01Open: eight hundred milligrams is a high-step tablet
Open on the 800 mg scored tablet. Annotate saturable absorption and the CrCl table. Countersign the opioid respiratory warning. Seal with the ninety-count fill.
Neurontin tablets are 600 mg and 800 mg. Capsules are 100, 300, and 400 mg. Solution is 250 mg/5 mL. This despatch locks the 800 mg tablet, ninety count. Day-one PHN is a single 300 mg dose, not an 800 mg swallow 'to get ahead of the pain.'
Epilepsy in people 12 and older starts at 300 mg three times a day and may titrate to 600 mg three times a day. The 800 mg tablet is how some charts reach 2400 mg/day (800 three times) once the gut and the kidneys have been counted. The longest gap between epilepsy doses should not exceed 12 hours.
Costco, Publix, Sam's Club, and Target sit on this letter's store cards. Ask for the high-step tablet, not the 300 mg capsule, when the script is 800 mg. Juvia does not dispense.
02Fourteen percent food bump is not a meal-hold rule
Food raises gabapentin AUC and Cmax about 14%. That is a slight effect. Take the 800 mg tablet with or without food and keep the times stable. Chasing a 'boost' by pairing every dose with a feast is not labeled strategy.
The antacid gap is the real meal-adjacent rule: at least two hours after aluminum-magnesium antacid. Reflux season is when people forget and then call the tablet weak.
Saturable absorption already means 800 mg three times a day is not three clean 800 mg deliveries. Adding food theatre on top just confuses the log.
If nausea on an empty 800 mg is the complaint, a small snack is reasonable. If the complaint is 'it does nothing unless I eat a steak,' look at CrCl, the opioid stack, and whether 1800 already failed for PHN before anyone adds a fourth 800 mg.
03What this tablet does not do, and what people still stack
Gabapentin is not an SSRI and not a wake tablet. Stacking it with fluoxetine 20 mg for 'nerve pain plus mood' is two indications, two adverse-effect maps. Sedation can deepen. That can still be reasonable. It is not automatic.
Off-label use for anxiety, hot flashes, or alcohol withdrawal is common in clinics and is not the Neurontin PHN or epilepsy claim. This letter will not invent a dose for those uses. If your 800 mg tablet is for an off-label aim, the CrCl cut and the opioid warning still apply.
Pregabalin is a sibling gabapentinoid with a different label set (including diabetic neuropathy and fibromyalgia on some US labels). It is not this 800 mg lock and not a 1:1 milligram swap.
04Ninety high-step tablets, dated cash, no capsule quote
GoodRx tablet table, 19 July 2026: generic gabapentin 800 mg x 90 at $104.59 average retail and $22.67 with a coupon. That is the caption on this lock. A 300 mg capsule ninety-count is a different line. Do not compare them to claim the 800 mg tablet is 'overpriced for the same drug.'
Publix hrefs go to Publix pharmacy, not to GoodRx. Costco, Sam's, and Target the same way. Coupon math stays in the caption.
If CrCl later falls, the bottle of ninety 800 mg tablets may become the wrong unit even if the cash was fair in July. Re-file the renal step before the next refill, not after a foggy week.
05Creatinine clearance writes the daily ceiling
When CrCl is 60 mL/min or higher, the labeled total daily range is 900 to 3,600 mg. Example regimens in that band include 300, 400, 600, 800, or 1,200 mg three times a day. An 800 mg tablet three times daily is 2,400 mg, inside that band, not a license to ignore dizziness.
CrCl 30 to 59: 400 to 1,400 mg/day. CrCl 15 to 29: 200 to 700 mg/day. CrCl under 15: 100 to 300 mg/day, and if CrCl is 7.5, use about half the daily dose used at 15. These cuts override a pain score that 'still needs 800 three times.'
Hemodialysis: keep a maintenance dose from the CrCl estimate plus a supplemental dose after each 4-hour dialysis run. Elderly charts often hide a CrCl of 40 behind a 'normal creatinine.' Cockcroft-Gault belongs on the note before the 800 mg tablet is chronic. The short file is renal steps.

06PHN titration lands at 1800 before anyone needs an 800 mg split
Adult postherpetic neuralgia: day 1 one 300 mg dose; day 2 600 mg (300 mg twice); day 3 900 mg (300 mg three times). Then titrate as needed to 1,800 mg/day (600 mg three times). Efficacy in studies showed across 1,800 to 3,600 mg/day. Extra benefit above 1,800 was not demonstrated.
So the 800 mg tablet is a later convenience for a high daily total, or a way to build 2,400 mg, not a day-three shortcut. People who swallow 800 mg on night one invent ataxia they then blame on 'Neurontin sensitivity.'
Epilepsy adjunct, age 12 and up: start 300 mg three times daily; maintain 300 to 600 mg three times daily. 2,400 mg/day was tolerated in long-term work; 3,600 mg/day in small, shorter groups. Children 3 to 11 use mg/kg rungs (10-15 mg/kg/day start; targets 40 mg/kg/day at ages 3-4 and 25-35 mg/kg/day at ages 5-11). This letter does not lock a pediatric 800 mg tablet.
07Dizziness boards, driving, and the 12-hour epilepsy gap
Somnolence and dizziness were common enough in the labeled trials that the first week on a new high-step total is a poor week to test night driving. 19% somnolence versus 9% placebo in the epilepsy board above age 12; 21% versus 5% in PHN. Those are not rare-event footnotes.
Ataxia at 13% versus 6% in that epilepsy board is why night-one 800 mg is a bad idea. People call it 'I am sensitive to Neurontin' when they skipped the 300 mg build.
Epilepsy dosing three times a day with more than 12 hours between doses raises seizure risk. Two 800 mg tablets at 22:00 to 'catch the missed noon' is the pattern the 12-hour rule exists to stop.
Suicidal thoughts are an antiepileptic-class watch. New hopeless talk after a gabapentin start needs the same-week call you would make for an SSRI, even though this is not fluoxetine.
08Dialysis days and why pediatric 800 mg is not this lock
Hemodialysis removes gabapentin. The label wants a maintenance dose from the CrCl estimate plus a supplemental dose after each 4-hour run. Taking the 800 mg tablet 'whenever I remember' on a dialysis week is not that plan. Write the post-HD dose on the same card as the chair time.
Pediatric renal dosing was not studied. Children 3 to 11 with partial seizures use mg/kg rungs, not an adult 800 mg tablet broken by eye. Neuropsychiatric reactions in that age band are a labeled watch.
Anaphylaxis and angioedema: swelling, trouble breathing, stop and emergency care. A mild itch after the first 300 mg is a clinic question; facial swelling is not a 'wait and see' on an 800 mg night.
Cimetidine at 300 mg four times a day dropped gabapentin oral clearance about 14% in a small study, in step with creatinine clearance. The label did not treat that as a major interaction. It is still a reminder that renal drugs travel together.
09Coming off 2400 mg is a written taper, not a Friday cleanse
Chronic high-step totals should not stop cold. Pain can rebound. Anxiety and insomnia can appear. Epilepsy charts can seize. Ask for a stepdown that names each 800 mg removal and the days between steps.
A person who 'feels clear' after missing two days is seeing withdrawal as well as relief. That is not proof the drug was only sedation. Recheck CrCl before restarting the old 2400 if the miss was a hospital week with a rising creatinine.
Leftover ninety-count bottles after a taper are not for a neighbor with shingles. The 800 mg tablet is the wrong starter for that neighbor. Return unused tablets to a take-back, do not mail them.
If opioids leave the chart first, gabapentin may still need its own exit. Dropping both the same Friday stacks two withdrawals. Sequence them with one prescriber.
10Scored 800 mg tablets: use the other half next, discard after 28 days
If you split a scored 600 mg or 800 mg Neurontin tablet, take the unused half as the next dose. Half-tablets not used within 28 days of the split should be discarded. A pillbox of crumbling halves from last month is not a legal '800 mg lock.'
Swallow with or without food. Three-times-daily epilepsy dosing with a gap longer than 12 hours is a seizure-risk pattern. High-step tablets make that gap easier to invent ('I will take 1600 at night'). Do not.
Withdrawal after chronic high-step use can include anxiety, insomnia, pain rebound, and in epilepsy, more seizures. Taper with the prescriber. A weekend 'cleanse' off 2,400 mg is not a taper.
11Opioids plus gabapentin: respiratory depression is on the label
Respiratory depression can occur with Neurontin plus CNS depressants, including opioids, or when the lungs are already impaired. Monitor and cut doses. This is a warning, not a 'nice to have' caution.
Dizziness and somnolence are the common reasons people stop. In epilepsy trials above age 12 (up to 1,800 mg/day), somnolence was 19% versus 9% on placebo, dizziness 17% versus 7%, ataxia 13% versus 6%. In PHN trials (up to 3,600 mg/day), somnolence 21% versus 5%, dizziness 28% versus 8%.
Alcohol on an 800 mg evening dose is the same sedation stack in plain clothes. The sedation postscript is the short file. Do not drive until you know how the high-step tablet sits. Some US states schedule gabapentin; federal US law does not put it in the same CSA box as modafinil. Local rules still bind the window.
Stops and watches that outrank a pain-score argument

12Saturable gut uptake: more milligrams buy a smaller percent
Gabapentin bioavailability is not dose-proportional. About 60% at 900 mg/day, 47% at 1,200, 34% at 2,400, 33% at 3,600, and 27% at 4,800 mg/day, each in three divided doses. An extra 800 mg tablet does not add an 800 mg-shaped piece of plasma.
Food raises AUC and Cmax about 14%. That is a slight effect, not a meal-hold like some other neurology drugs. Aluminum-magnesium antacid (Maalox) cut mean bioavailability about 20%. Take gabapentin at least two hours after that antacid.
The drug is not metabolized by CYP enzymes in any way that writes a ketoconazole story. It leaves unchanged in urine. That is why the kidney paragraph below outranks a CYP paragraph on this page.

13Seal: high-step tablet, kidney first, opioid second
The lock is generic gabapentin 800 mg tablets x 90. PHN still starts at 300 mg. Extra benefit above 1,800 mg/day was not shown. CrCl under 60 cuts the ceiling. Half-tablets die at day 28.
Opioid stacks get a respiratory watch. Dizziness rates on the labeled boards are high enough that 'I will drive tomorrow' needs a day of data. Method: Open, Annotate, Countersign, Seal.
